Applicants from an non-accredited school who have received a positive eligibility evaluation may apply for the National Dental Hygiene Certification Examination (NDHCE) by submitting the following documents before the application deadline:

• NDHCE Application Form (online or printable version)
• Appropriate fee + tax
• 2 pictures - passport size (or upload it using online application)

Copies of your official transcript and your diploma are not required since we already have all we need on file.

Please note that Applicants must meet all Eligibility evaluation criteria before they can apply to write the NDHCE. If those criteria are not met, the NDHCB will not accept the application but will reimburse the application fee minus a non-refundable fee of $100 (+ tax) , and the applicant will be denied the opportunity to write the NDHCE.
